My Patient's Health Plan

The health-plan information available to you through this site summarizes your patient's medical benefits based on your patient's participation with a specific plan. If you have any questions regarding the information, you may consult the Evidence of Coverage for your patient's plan, contact Customer Service at the number listed on the member's ID card or email your question to Customer Service.

To view the benefit information, enter the patient's ID number from the Health Advantage membership card.

Verify you have entered the number exactly has it appears on your ID card.
